Adult Care Centers

Does your loved one need a safe and caring place to stay during the week while you work or attend to other responsibilities? JABA’s Adult Care Centers in Charlottesville and Louisa provide a full day of fun, caring staff and intergenerational activities and outings. Transportation may be arranged through outside sources such as JAUNT. We specialize in person-centered care for the aging (including Alzheimer’s/dementia), but also welcome disabled adults. Call to learn more about rates/scholarships and to arrange a visit.

These centers receive scholarship support from local governments and individual donors. This is a JABA, Inc. service.

Some forms you'll need to fill out to attend the Adult Care Centers:

Application Form- Used as our member’s face sheet of their medical chart which allows us to have quick access to personal information.

Nutritional Assessment Form - Used to determine if our clients are at risk prior to attending at our center.

History and Physical Form [For Charlottesville / For Louisa- Completed by primary care physician, which covers current medications, diagnosis, and TB test. This form has to be completed within 30 days of starting our program. 

Family Assessment Form - Caregiver’s understanding of their loved ones functional status (ADLs-activities of daily living and IADLs- Instrumental Activities of Daily Living) prior to attending our center.
